Shenley FC COVID-19 Team Track and Trace

Shenley FC has reviewed and followed the FA and Government guidelines for our members to return to footballing activity. The safety of all players, parents/carers and coaches is paramount and everyone involved with grassroots football must play their part to ensure we all adhere to the FA and Government guidance for the phased return of sport.

As per Government guidance and to support NHS Track and Trace, a register will need to be kept of every member in attendance and held for 21 days.

​

Our SFC teams will not provide a list of attendees as this is submitted to our Club COVID-19 Officer (Gary Barrett) and held at club level to manage our members information.

​

Our team manager will act as the 'lead contact' of the SFC group as will the opposition manager for the all visiting players, and will provide their contact details (name and telephone number) for your club to keep on record along with our Club COVID-19 Officer's details if required.

COVID-19 GUIDANCE FOR MATCHES – VISITING TEAM— Before leaving your home:

​

  • SFC COVID-19 form Confirmation to be completed by visiting team manager.

  • Everyone to self-assess for COVID-19 symptoms (coaches, parent/carer and child - if they want to tell us, please do by filling in the individual track and trace form here).

  • Ensure all players bring their own small hand sanitiser for personal use along with a drink.

  • Coaches to ensure they have their own disinfectant for use prior to, during and after the match.

  • Players to be ready to play upon arrival as there will be no changing facilities at the venues.

     

Arriving at the venue:

  • Coach to set up and organise a ‘team area’ on the coaching side of the pitch. Also disinfect all equipment that will be used (cones/footballs).

  • Players to arrive on site and head towards their coach who will advise on the next steps.

  • Parent/carer to observe social distancing in groups of no more than 6 on the side of the pitch behind the ‘respect line’

During the match:

  • Coaches, players and match officials to sanitise their hands before, during breaks and after the match.

  • Coaches and substitute players to remain in their ‘team area’ and not cross over to the oppositions side.

 

At the end of the match:

  • Ask the players to collect their belongings and then go to their parent/carer.

  • Please do not congregate after the match as other teams will be using the area.

By completing this form you are consenting for your team to take part, you are confirming that you have read and understood the activities being offered to your team and agree with the measures the club has put in place to manage any risks, including its Covid-19 measures in line with current Government guidance. A copy of the club’s risk assessment/management documentation can be found on our website; https://shenleyfc.wixsite.com/shenleyfc/documents.

If any of your teams children have any specific medical conditions, please ensure that you discuss these with your club welfare officer and or coach and agree the best way to support their needs. If you wish to withdraw consent to your team(s) participating in any or all activities, please notify us in advance, or as soon as possible via our contact page. If your child has had COVID-19, or symptoms for more than seven days, you must seek medical approval from your family doctor before they can restart any football activity.

DECLARATION

Each participant should self-screen prior to arrival at training to ensure they do not have any of the following symptoms (confirmed by a parent for those under age 18), as these are potential indicators of Covid-19 infection and confirm they check the conformation box on the above.
