Shenley FC COVID-19 Team Track and Trace

This form is to be completed by the visiting team manager for every fixture played at a Shenley FC location. The information collected will not be shared outside of the club or used in any way other than to support the NHS Test and Trace. We also have an individual details form your team members and supporters can complete if they wish.
The purpose of this form is to confirm you have read the FA and Governments guidance in relation to 'the phased return of sport' and will act as the 'lead member' for all people attending on behalf of your team.
As per Government guidance, a register will need to be kept of every member in attendance and held for 21 days. Our SFC teams will not provide a list of attendees as this is submitted to our COVID-19 Officer (Gary Barrett) and held at club level to manage our members information.

We will take a similar approach with visiting teams in asking that the team manager act as the lead contact of the group for the all visiting players and that we will liaise with the manager should an issue arise.

By completing this form you are consenting for your team to take part, you are confirming that you have read and understood the activities being offered to your team and agree with the measures the club has put in place to manage any risks, including its Covid-19 measures in line with current Government guidance. If any of your teams children have any specific medical conditions, please ensure that you discuss these with your club welfare officer and or coach and agree the best way to support their needs. If you wish to withdraw consent to your team(s) participating in any or all activities, please notify us in advance, or as soon as possible via our contact page. If your child has had COVID-19, or symptoms for more than seven days, you must seek medical approval from your family doctor before they can restart any football activity.

DECLARATION
Each participant should self-screen prior to arrival at training to ensure they do not have any of the following symptoms (confirmed by a parent for those under age 18), as these are potential indicators of Covid-19 infection and confirm they check the conformation box on the above.
